No, seriously though, how many missions do you have to complete in order to get past the first island?

It\'s not too many. It takes awhile because you\'ll open a ton of stuff to do as distractions from the main quest, but you\'ll be heading out toward Fierro sooner than you expect. It just kind of happens out of nowhere.

It unlocks the second island when you have completed the cop missions at the donut shop.
